We have had cars for over 100 years, but Rome has been around for thousands of years! Therefore, the streets aren't all exactly big enough for normal cars. This is why small cars and motor scooters are so popular. The trams run through all the larger streets which cuts down on the need to have a car or scooter at all. How would you get around Rome?
